If you have experienced a workplace injury you must report a workers compensation claim to start the process to receive service, support, and timely treatment.

Report a claim soon as possible. Our online reporting form will guide you step-by-step and prompt you for information as you go. You will be asked to provide information related to the claim as you go through this process.
